Workplace Hazard Assessment (WHA)

The Occupational Safety and Health Administration (OSHA) requires employers to conduct inspections of all workplaces or tasks to determine if hazards are present that would require the use of Personal Protective Equipment (PPE). The Workplace Hazard Assessment Form (WHA) has been designed to help UConn supervisory personnel choose appropriate PPE for their employees and provide the written certification required to comply with the OSHA regulation (29 CFR 1910.132 Subpart I).

WHA forms are available for both General Workplaces and Laboratories. If you are unsure of which form to complete, contact EHS for assistance (860-486-3613). A completed form must be submitted to EHS and a copy must be kept at each work location and made readily accessible to employees. Completed forms must be sent to EHS via email to ehs@uconn.edu, FAX 860-486-1106 or mail to Unit 4097.

The next step after completing a WHA is to fill out the  ESTA to determine which safety training, if any,  is needed to conduct work in that space.
