The Occupational Safety and Health Administration requires employers to conduct inspections of all workplaces or tasks to determine if hazards are present that would require using Personal Protective Equipment (PPE). This process is carried out through a Workplace Hazard Assessment to guide the selection of appropriate PPE. A written certification must verify that a Workplace Hazard Assessment (WHA) has been completed.
UConn supervisory personnel (including Principal Investigators and designated supervisory staff) must complete a WHA form to comply with this OSHA requirement. Completed forms must be submitted to EHS and a copy kept at the work location and made readily available to employees. Select an appropriate Workplace Hazard Assessment Form applicable to the workplace to be assessed. If you are unsure of which form to use, select the form for general workplaces or contact EHS for assistance at 486-3613.
